Education Fund Post Project Report

Your Education Fund post project report is due within three months of completing a program. If funding covered a program over several years, please submit an interim report no later than March 31, 2011 and a final report within one month of completing the program. If your library was funded for several different programs ending at different times, please submit multiple reports.

Note: If the form session is idle for 20 minutes the form will fail. Typing data in a text box is considered 'idle'. It is recommended you gather your data before starting to fill in the form. You cannot fill in part of the form and return to it at a later time.

Budget Reconciliation
Please update the approved budget (sent with the letter approving your grant) by completing the Actuals column in the Budget Reconciliation Form. Once this is done, you can upload your file via the form at this link.

If a final Application Program and Education Fund Reimbursement Request Form has not yet been submitted please forward that along.
