Basis for Agreement |
This CPA agreement is a result of a Request for Proposal in
the summer, 2004 to secure a benefits consultant to a) assess the feasibility of
establishing a consortium approach to group benefits for public libraries, and
if feasible to b) select an insurance carrier and fill the ongoing role of
broker. |

| General Terms and Conditions |
Libraries interested in providing benefits to employees, or
interested in comparing prices against their current benefit costs (whether
purchased alone or as part of the municipality) should contact Mosey and Mosey
directly. Coverage can be tailored to the specific library’s needs if nine or
more staff is insured. For smaller libraries, membership is in a pooled plan.
Coverage is normally for one year at a time. Please refer to the Mosey and Mosey
