Instructions for the use of the Inter-branch Delivery form

  1. Please download and print a copy of the Inter-branch Delivery Form
  2. On the designated days of Inter-branch pick up, library staff are requested to count the number of boxes the SOLS driver will be picking up and record this number on the appropriate line of the form.
    1. MAIN Br. Staff - Please remember this does not include ILLO material.
    2. Large and over size boxes will be counted as 2 items. A large box is anything over 21 inches long X 13.5 inches wide X 6.75 inches deep, with a maximum weigh of 50 pounds.
  3. SOLS drivers are to verify the information provided by the library staff.
    1. If there is a discrepancy, the SOLS driver will contact library staff (whenever possible) and/or record the correct number of boxes.
  4. The SOLS driver will pick up this form on the last day of each month and forward it to the Supervisor, Delivery Service.
    1. Library staff will be responsible for making any copies of the form that they require.
    2. SOLS drivers will maintain a running total of the number of boxes picked up.
  5. Library staff will start a new form each month.
  6. SOLS will invoice the library as per the Letter of Agreement for the service.
